Social Workers

Our licensed clinical social workers serve as lead care managers for many of our members. They are skilled in assessing psychosocial needs, providing counseling, and connecting members to community resources. What they do: Social workers help develop each member’s care plan and coordinate services like housing assistance, food programs, or support groups to address social determinants of health . They advocate on behalf of members – whether it’s with a landlord, a government agency, or a healthcare provider – to make sure our members’ needs are met with dignity and respect. Perhaps most importantly, our social workers build trusting, one-on-one relationships with members and their families. They offer emotional support and guidance through challenges, empowering individuals to take positive steps toward their goals.

Registered Nurses (RNs)

Our RNs bring medical expertise and clinical care coordination to the team. They focus on the health care aspects of each member’s plan. What they do: Nurses educate members about their health conditions and medications, helping them understand doctor’s instructions and make informed decisions . They coordinate care with hospitals, clinics, and specialists – for example, ensuring a member’s discharge plan from the hospital is followed up on, or communicating with a primary care physician about any changes in the member’s status. RNs also provide health coaching, helping members manage chronic diseases like diabetes or hypertension with lifestyle changes and medication adherence . If a member has symptoms that need attention, our nurses triage those concerns and arrange timely medical care. They are critical in bridging the gap between the member and the healthcare system, making sure nothing is overlooked medically while we address social needs.

Case Managers / Care Coordinators

Our case managers (also referred to as care coordinators) are the organizers and problem-solvers who keep each member’s care on track. What they do: They handle the logistics of care – scheduling appointments, arranging transportation, helping with insurance issues, and following up on referrals. Case managers maintain regular contact with members, often checking in by phone or in person each week to see how things are going. They track progress on the care plan and update it as needs change. A big part of their role is communication: they act as the hub between the member, their doctors, health plan, and any community service providers . For example, if a member needs to see a specialist, a case manager will ensure the referral is made, the appointment is set, transportation is confirmed, and any required paperwork is done. They also troubleshoot obstacles – if a member misses an appointment or loses contact, our care coordinators will reach out persistently (even visiting in person) to re-engage them. Our case managers are compassionate, persistent advocates who make sure all the pieces of a member’s care come together smoothly.

Peer Counselors (Peer Support Specialists)

Peer counselors are individuals with lived experience who are now using their experiences to help others. They might be someone who has managed their own mental health recovery, overcome substance use, or successfully transitioned out of homelessness or the justice system. We include peer support specialists on our team because they offer a type of understanding and hope that only someone who’s “been there” can provide. What they do: Peer counselors mentor and motivate our members. They might accompany a member to a new support group to help them feel comfortable, or share personal insights about staying on track with treatment. Peers help members set achievable goals and celebrate milestones, no matter how small. They often assist with teaching life skills – for instance, how to budget groceries or use public transportation – side by side with the member. Perhaps most importantly, peer counselors build rapport through empathy and shared experience. For a member who is feeling distrustful or discouraged, talking to someone who has walked a similar path and come out the other side can be incredibly inspiring. Our peer counselors embody the message that recovery is possible and no one has to go through tough times alone. They are an integral part of our team’s ability to engage members and keep them involved in their care.

(In addition to the roles above, our extended team includes program managers, outreach workers, and administrative staff who support the ECM program’s operations. Every person at CCHC, whether in the field or behind the scenes, is committed to our members’ success.)

Together, our team brings a wealth of knowledge and compassion to the table. We hold regular team case conferences, share insights, and coordinate our efforts so that each member gets well-rounded support. When you enroll in our ECM program, you’re not just getting one care manager – you’re getting an entire team of people working collaboratively to help you achieve your health and life goals. We’re proud of our team and the caring professionals who make CCHC a trusted partner in our community.
